You are able to turn on an option that would ask parents to enter medical info for their students when they are creating their accounts. This is part of your single signup form on your parent portal.
To turn this option on, go to More > Settings, and click "Portal Settings" in the left menu. Then click the "Options" tab.
On the "Options" tab, check the box to "Ask for Medical Details." Then check the boxes for the fields you would like the parents to fill in. If you want them to enter info for all four then check all four boxes.
Now when a parent is setting up their account they will be asked to enter info for the medical fields you selected.
